Recording Safety Observations in O3

This article discusses how to record Safety Observations using our web-based app O3. Click here to learn how to record Safety Observations using our iPad app O3Go.

To begin, navigate to Project Information > Safety Observations at the top of the page.

Doing so will take you to the Safety Observations grid. To add a new Safety Observation, click on the + icon in the top-right corner above the grid.

This will bring up the Safety Observation form. At the top of the form, users can fill in fields pertaining to the Observation.

At the bottom of the form, users can indicate whether the observed were safe or at-risk for all the given Safety Observation Types by clicking on the expandable list. By default, each Type will be set to N/A.

If the observer had to intervene for any at-risk behavior, they can indicate this by clicking the Intervention checkbox.
